Learning collaboratives are designed to help accelerate improvement in the substance use disorder/opioid use disorder (SUD/OUD) system of care for Delawareans and serve as a way for providers to network and build relationships. Each of the learning sessions is designed with an emphasis on adult learning principles, interactive training methods, peer-to-peer teaching, and skill-focused learning. Due to the COVID-19 pandemic and social distancing protocols, collaborative learning activities have been held virtually through an online zoom application.

Extension for Community Healthcare Outcomes (ECHO) series
Social Determinants of Health (SDOH) contribute to wide health disparities and inequities. Social and physical determinants affect a wide range of health, functioning, and quality of life outcomes. Poor health outcomes are often made worse by the interaction between individuals and their social and physical environment. Social determinants are in part responsible for the unequal and avoidable differences in health status within and between communities.
Unmet social needs often prevent individuals from accessing, navigating and sustaining the recovery process. Therefore, DSAMH’s goal is to address the social determinants that account for 80% of overall health outcomes through an integrated, community-based system of recovery support with the result of improved well-being for the individuals we serve and thriving, resilient communities. Listed are the SDOH identified by DSAMH and the plan to address each.
- Housing – Continuum of affordable, integrated, supported, and independent housing.
- Peer Support – Workforce development training and technical assistance for certified peer recovery specialists and peer supervisors.
- Community inclusion – Community outreach; holistic case management; mainstream activities with meaningful purpose.
- Employment and education – Job training and placement; Access to support such as childcare, funding and transportation.
- Primary Care – Access to a physical health practitioner
- Food Security – Access to healthy food; counseling and dietician support on making affordable healthy meals.
- Transportation – Free and timely access to safe and reliable transportation.
- Legal – evidence-based, no cost, Medical-Legal Partnership
DSAMH Social Determinants’ initiatives are aligned with the strategic goals of Delaware’s Behavioral Health Consortium (BHC)and SAMHSA. The alignment also provides greater opportunities for broad, system-level change and sustainability of DSAMH Social Determinants’ initiatives.

Delaware Treatment and Referral Network (DTRN) is a comprehensive referral network to help manage transitions of care for behavioral health and substance abuse treatment in the State of Delaware. DTRN automates the current referral process using a web-based application that securely connects Delawareans to available Behavioral Health care.

Treatment Connection is a resource available to all Delawareans and providers. It allows you to search for treatment facilities by location and filter the results based on specific need. You can learn about treatment options and take an interactive assessment to see which options might be right for you or your patient. Treatment Connection will provide access to:
- Decision support self-assessment tool to assist in determining the most appropriate type and level of treatment needed
- Searchable list of trusted treatment services and providers by zip code and distance
- Collection of resources and educational materials
- Provider list with an up-to-date treatment capacity
- Secure messaging to communicate with treatment providers
In 2019, DSAMH increased access to services and transitions by opening Bridge Clinics in each county to provide screenings, referrals to treatment and social support. In doing so, providers have been able to implement rapid access to individualized evaluation, transitional care planning and therapy.
To further uphold DSAMHs mission to strengthen access to care, support care transitions and stabilize individuals with behavioral health needs, Bridge Clinic services in New Castle, Sussex and Kent Counties were extended to 24-hours/7 days a week, serving as a pathway for stabilization from Emergency Departments across the State. DSAMH also provides transportation from Emergency Departments at all hours, ensuring services can be provided in an expedited manner.
To increase outreach efforts, DSAMH has expanded the Bridge Clinics to Mobile Bridge Clinics and Mobile Pantries in partnership with community organizations to distribute food, personal protective equipment (PPE), naloxone and offer on-the-spot counseling.

Eligibility and Enrollment Unit (EEU) functions as the “gatekeeper” for the Division of Substance Abuse and Mental Health (DSAMH). There are two distinct pathways to engaging with EEU:
- Hospital placement for inpatient psychiatric hospitalization
- Long-term care services range from residential treatment to community continuum of care programs
The EEU evaluates each individual’s need for these services based on a variety of criteria. When the EEU determines that a person is eligible for hospitalization or long-term care services, they authorize a specific quantity of services and link the eligible person to a provider.
Psychiatric and substance abuse hospital admissions involve the EEU and receives documentation for all 24-hour detainments throughout the state. The EEU reviews documentation for accuracy, appropriateness and verifies insurance for each client. DTRN is utilized to determine bed availability, send referrals, and serve as a liaison by facilitating communication between referring providers and psychiatric facilities.
Brief screens take place via the EEU and are used to access PROMISE services. The EEU receives applications from community providers, IMDs and MCOs for consideration for PROMISE. Clients can also call 302-255-9460 to self-refer for a screening over the phone. During the referral process, the Brief Screen staff may provide alternative resources if presumptive eligibility is not met, or other services are more suitable to meet the client’s needs.
